Whether you’re coming home from a hospital, rehab, or nursing home, noticing a change in your health at home, or your parent is in assisted living and needs a nurse, it can feel overwhelming to handle everything alone. 💊💙

The NextGen Bridge Program offers free nurse visits at home to support people in the Boston, North Shore, and South Shore areas.

👩‍⚕️ 1–3 nurse visits per week
🗓️ For 1–4 months
💊 Medication teaching
💉 Injections at home (such as insulin and other weekly or monthly injections as prescribed)
♻️ Chronic condition teaching and assessment for Diabetes, CHF, COPD, and high blood pressure (HTN)
🧭 Teaching for patients, family, and caregivers on what to watch for and when to call for help
💙 No cost for the nurse visits (insurance may be needed for medications or supplies)

We coordinate your care with your primary doctor and specialists, so everyone stays on the same page.
